How We Measure Arthritic Changes on X-Ray

Spinal disc model showing the stages from a healthy disc to bulging, herniated and degenerated discs

“Do I have arthritis in my spine?” Your X-rays can answer that better than almost anything else, and the answer usually isn’t as scary as people expect. Here’s how we look for arthritis on X-ray.

What we look for

On every spine X-ray, I check each vertebra for normal contour and density. A healthy vertebra has smooth, even edges and a consistent shape. As a joint takes extra stress over time, the body responds by laying down extra bone. On X-ray, that shows up as:

  • Bone spurs (osteophytes): small lips or beaks of bone along the edges of a vertebra.
  • Changes in shape and density: edges that look sharper, flatter or denser than the levels around them.
  • Narrowed disc spaces: less room between two vertebrae, which often goes hand in hand with arthritic change.

Why arthritis isn’t random

Arthritic changes tend to show up where a joint has been loaded unevenly for a long time. That’s why we always look at the position of your spine alongside its condition. A neck that has lost its curve, for example, puts more pressure on the front of the discs and joints, and that’s often right where we see the changes.

What it means for you

Seeing arthritis on an X-ray doesn’t mean you’re stuck with pain. Plenty of people with arthritic changes feel great. What it does is help us tailor your care, choose the right techniques for you, and set realistic goals. Learn more on our arthritis and spinal degeneration page.
